Illustrative service examples

What your service documents can look like

Fictional examples of a booking outcome, visit preparation, fee breakdown and non-clinical handover.

Sample 1: a booking outcome

Fictional example — no appointment has been booked. A real outcome records the hospital and campus in English and Chinese, the department, date, official confirmation source, check time and next action. If the agreed target is not secured, that result must be stated clearly.

Sample 2: a visit preparation sheet

Fictional example — use your own hospital's instructions. The sheet records the confirmed meeting point, coordinator contact, arrival time and documents the hospital has requested. Questions about fasting, medication or tests go back to the clinician; our team does not invent clinical instructions.

Sample 3: a service fee breakdown

Illustrative calculation, not a personalised quote: half-day support is $249 USD in total. If $49 USD appointment coordination has already been paid for the same visit, the remaining service fee is $200 USD. Full-day support is $399 USD in total, or $350 USD remaining after that same credit. Hospital costs and approved extras are separate.

Sample 4: the written handover

Fictional example — not a medical report. A service summary records the practical work completed, documents actually received, unresolved administrative questions, the person responsible and the next agreed date. Clinical statements must be attributed to the hospital or clinician, never filled in by the coordinator.
